it can make about 3-5° difference on a bare cpu like an old athlon, but on new lidded cpu's best result is take the lid off and reseat it with artic silver then use it again between lid and HSF. Dont use too much though it is highly electrically conductive and a tiny bit will short out shitloads

I use ceramique anywhere where there could be shorts as there's >1 degree difference to silver especially if its thin.
Haven't delidded any quads or core 2 duo's but athlon64 & p4/p4d lids were stuck down with silicon rubber. Best method is a very fine surgical scalpel blade (now where would a guy who works in a hospital get those?) Personally don't think it's necessary unless you're extreme overclocking, and Artic silver on the outside of the lid is just a waste.

I've got a 9850be which has a maximum temp rating of 60 degrees, I'm running a scythe ninja 2 with 120mm fan to cool it and even though the overall temp (which I think is the rating one) only gets to 53 the individual cores peak at 61 after 20 mins of full load. I'd like to try clocking it quite a bit higher than the current 2750Mhz but will probably need to up the core volts a bit for that which will again up the temps
So ideally I'd like to do away with the heatspreader completely and water cool directly on the cores, But, since I can't afford wc or a couple of replacement chips, I reckon fitting the spreader properly will give me another 6-8 degrees to play with (it has on previous amd chips) but I've never stripped a quad before

You can lap the cpu and the Ninja to lower the temps maybe?
I takes a couple of hours to do it properly but what i've read about it you can surely take of 5/6 degrees

Do NOT try removing heatspreaders from quad core chips. They are soldered to the die and will kill the chip if forced.
Apparently they can be removed by careful use of a hot air gun but this has a 25% failure rate
Still removing the rubber seems to have given a better seat on the IHS and switching from the Ninja to a Zalman 9700 has the cores down to 51 degrees full load and cpu case temp down to 46
